Attention Erasmus + students

Documents needed to complete the subjects during the trip to university are:

1. Certificate of settlement of the departure from the International Cooperation Department of the PP (for persons ending the trip),
2. signed by both universities and the student Learning Agreement (optionally with the document Changes),
3. signed Transcript of Records (grades and credits for subjects studied abroad,
4. completed Student Periodic Achievement Card (KARTA OKRESOWYCH OSIAGNIEC STUDENTA wzór) – 2 copies, the card should be filled in after returning to the country;
5. Item cards (one A4 for each item, pattern, please remember to include graphic information, e.g. reduced charts),
6. Table comparing program - formula

If it is not possible to obtain these documents within the given deadline, an application for an extension of the session should be submitted
(it is possible to send a signed and scanned document to the address of the dean's office: Dean's Office of WA).
